When designing a data collection tool, always try to make it simple yet versatile to collect as much information as possible. Avoid complex and ambiguous wordings that would would bring a lot of misinterpretation. Also try to know the leve of education of the intended users of the tool, this would necessitate a creation of a user friendly tool.

@Dmegastar I work for a school meals organization and we use these types of tools to collect enrollments and attendance. We use the teachers to collect this data.

I am currently studying but previously worked for an international Ngo Plan International. This is a global child centred organisation that does a lot of surveys, focus group discussions and data analysis to gather information related to children's education, healthcare and welfare of their families. They work with sponsored families based in the west that normal request detailed information about the children , their families and the community.

plan normal design questionnaires and in other to obtain the necessary and relevant information it ensures that the tracking form is clear for community volunteers and staff to use It normally has the age , gender, community, occupation , literacy level for the participants and the name , signature and position of the person administering the form. All these is accompanied by the questions related to the subject of research. it is very important to get this correct because any mix up will result in a wrong analysis and conclusion of the subject matter.

That is another important aspect i tend to forget, that is. only information needed to measure the indicators should be included on the tools

When Creating data collection tools that users will be able to effectively use, it is important to note the users language, age and content. Tools need not to be complicated. The questions should be simple and easy to understand. The tools should also focus on getting relevant data for your project from the user.

@Agasha, For question 1, I don't think it a matter of trust, I think its important for participant to fill their own forms because it might motivate or give them confidence that they are fully integrated into the project or it could also be for confidentiality particularly in health or financial project where you need some critical or private data from participant.
Question 2. My experience from the previous organisation I worked for, to prevent field officer(enumerators) from manufacturing or filling the PTF themselves which according to plan was to be filled by participant, they normally asked us to take GPS coordinates of the exact locations where the forms were filled and also we each team have one supervisors as back-checker. This back-checker visit these communities after the team has gone and randomly select some participant to fill their PTF which he will bring to the officer for comparison. If we found out one or some PTF are totally differs from the field officer's PTF then that particular field officer is in problem and will be investigated and dealt with accordingly.

1. Data collection tools must be identified as simple and easy to understand with preferable in local language.
2. Questionnaire must be designed with simple way as anyone can follow it.
3. Use coding i:e Village : 1, Town: 2 or Barpada: BA

The first step is to identify, who the user is. This is the most important part of using data collection tools. @Agasha, For question 1, I don't think it a matter of trust, I think its important for participant to fill their own forms because it might motivate or give them confidence that they are fully integrated into the project or it could also be for confidentiality particularly in health or financial project where you need some critical or private data from participant.
Question 2. My experience from the previous organisation I worked for, to prevent field officer(enumerators) from manufacturing or filling the PTF themselves which according to plan was to be filled by participant, they normally asked us to take GPS coordinates of the exact locations where the forms were filled and also we each team have one supervisors as back-checker. This back-checker visit these communities after the team has gone and randomly select some participant to fill their PTF which he will bring to the officer for comparison. If we found out one or some PTF are totally differs from the field officer's PTF then that particular field officer is in problem and will be investigated and dealt with accordingly.

@Agasha these are great questions when considering a PTF, and @Alim16 thanks for addressing them! I've often wondered which way to go with completing forms like these... is it easier for a trained employee/volunteer to complete the form for the participant since they would be trained on how to complete it, or would it be better for the participant to complete it, even if that means follow-up questions when a form is incorrectly completed? I think the point you make about how completing a document like this themselves provides not only a sense of pride in their participation, but also security in the program itself is important to consider when making these decisions.

I think trust may not be the correct phrase but is an expression preventing the delegation of authority to others. I think is better when the respondent are allowed to express themselves in their own words . A field officer's word might express the information in a way that it was not intended to be .For me is a case of hearing the respondent directly.
In the second instance the use of photographic evidence avoids or eliminates the possibility of fraud . It is used by Plan International and I will recommend other organisations follow that route,

The second thing is the use of codes and instructions to accompany the data collection tool. Lastly, it is important to orient data collectors on the use of the data collection tool so that all of them are on the same page.

The best way to make sure that one is creating data collection tools that one's users will be able to use easily is by engaging in simple language and instruction methods, but also by testing one's tools with peers, the community, stakeholders, etc. This is done to gain their feedback and input on this matter, which in turn, will make the tool much stronger and clearer.

Some tips I could offer is to use simple words and offer a translated version of the tool. For example, if one is conducting research in Quebec, Canada, they might want to incorporate French in their tool in italics below or next to the English questions. Also, one could ask someone who fits that criteria to test the tool in order to gain their feedback and input on how to make it clearer or more understandable.

I agree with you @Bilha-Joy When language is simple, anyone (trained or untrained) will be able to understand how to use the tool.

To create a data collection tool that will be easy to use, there is need to consider a number of things

1. Who is going to use the tool? Is it the project staff who are conversant with the language used?, is it a beginner who does not understand M&E language, is it a participant who is going to fill the form?, do they need translation, how often is the data goind to be collected?
2. There is need to pre-test the tool before use. Pretest for question comprehension, is the question clear to everyone or it can be mis-interpreted, Is it measuring the indicator in question, Is it clear and not ambiguous,
3. Revise the tool regularly so that you make additions or remove things that will not be of help

Some essential tips to creating a data collection tool are
Understand your indicator and know what data you want to collect
Be very precise in the language and information on your tool
Use the language that is common to the people in the area. Translations most often are miseleading
Questions should have a series of choices the respondant should choose from.
